What are the Forms?

The Forms are used to submit requests or communicate information to the Oakwood Lakes Community Association or our Community Manager.  They have been created to be completed in both hand-written and computer-filled-in forms.  To complete a form by hand, please print the file and then write in the information needed.  Forms are used with our overall Governing Documents that include the CC&Rs, Bylaws, Articles of Incorporation, and Association Rules and Regulations.  For a more detailed explanation of the Governing Documents, please see the link below.

• Architectural Request Form (two versions)

• Official Exterior Paint Color Palette (Implemented 7/06/2006)

  The Oakwood Lakes Community Association has implemented an Official Exterior Paint Color Palette that is to be used for repainting homes in colors other than the original colors or previously approved color scheme.  The Paint Palette is documented in a book that is located in the offices of the Association's Community Manager, and use of the Paint Palette is defined in the Architectural Guidelines.  Representative illustrations of the color schemes and instructions for their use are also found on the Paint Palette page.

• Violation / Parking Complaint Form (two versions)

  NOTE:

  Please complete the entire form.  Incomplete complaint forms will not be processed.  Per Arizona law (A.R.S. § 33-1242 and A.R.S. § 33-1803), any complaint lodged with the Association will NOT remain anonymous.  The person complaining of the alleged violation must state their first and last name and this information will be sent, if requested, to the member who is accused of the violation.
